Support at Home Sunshine Coast: What the 2025 Aged Care Reforms Mean for You

If you or a loved one on the Sunshine Coast currently receives services through a Home Care Package or the Commonwealth Home Support Programme (CHSP), some big changes are on the horizon.

From 1 November 2025, the Australian Government will introduce the Support at Home Program — a new, streamlined way of delivering aged care for people who want to remain living at home. At Encompass Health Services, we’re already preparing for this shift, and we’re here to help you understand what’s changing and what it means for you.

Why Is Aged Care Changing?

The Support at Home Program is being introduced as part of the government’s response to the Royal Commission into Aged Care. The current system which includes HCP, CHSP and Short-Term Restorative Care will be merged into a single program to:

  • Simplify service delivery
  • Offer more personalised and flexible support
  • Improve transparency with pricing and budgets
  • Focus on helping people stay independent at home

What Does the Support at Home Program Mean for Sunshine Coast Clients?

If you’re already receiving aged care support at home, your services will transition to the new program from November 2025. You’ll continue to receive care but how your support is assessed, funded and reviewed will be different.

Here’s what to expect:

1. One Program, One Budget
Instead of package levels, you’ll receive a personalised budget based on your assessed needs. This allows for more flexibility and choice when selecting services like in-home allied health, personal care, or equipment.

2. Clearer Pricing
All aged care providers will publish standardised pricing for services, reducing confusion and eliminating hidden fees. You’ll know exactly what you’re paying for.

3. Stronger Focus on Reablement and Independence
The new model prioritises reablement, helping you stay mobile, active, and independent. Allied health services such as physiotherapy, dietetics, speech pathology and occupational therapy will play a greater role in achieving and tracking your goals.

4. A New National Assessment Tool (IAT)
A new Integrated Assessment Tool (IAT) will replace RAS and ACAT assessments, offering a more comprehensive look at your functional, physical, and social needs, and guiding funding decisions.

5. Ongoing Monitoring and Progress Reviews
Support at Home requires providers to regularly check in and report on progress. At Encompass, this aligns perfectly with how we already work — checking in, adjusting care, and making sure your needs are met over time.

How Allied Health Fits into the Support at Home Program

Allied health services remain central to keeping Sunshine Coast clients living independently at home. Under the new program, these services are recognised for their role in:

  • Reducing falls and hospital admissions
  • Supporting recovery after illness
  • Improving balance, strength, and mobility
  • Managing swallowing, nutrition, and cognition
  • Promoting confidence and participation in daily life

At Encompass Health Services, our team of allied health professionals will continue to work closely with clients, carers, GPs and support coordinators to ensure a smooth transition under the new system.

When Will the Changes Take Effect?

The Support at Home Program is expected to begin on 1 November 2025. If you’re currently receiving aged care services, your provider will support your transition. Clients on the Sunshine Coast can expect gradual changes, not a sudden shift, and ongoing communication throughout the process.
